COMMUNITY SERVICES NAVIGATOR

1 – FULL-TIME POSITION

Salary: $37.80 – $39.37 per hour  

(commensurate with qualifications and experience)

OPTIONS NORTHWEST is a community-based, non-profit, charitable organization dedicated to respecting the right of people to live life their way.  Through personalized OPTIONS, we collaborate with people and community partners to develop and offer creative personal and clinical services for individuals.

Position will require:

  • Diploma or degree in Social Work, Child and Youth Care, Developmental Services, Human Services, Psychology, Sociology, Education, Community Development, or a related field
  • Experience supporting youth, families, people with developmental disabilities, and/or individuals with complex support needs
  • Knowledge of transition planning, community development, service coordination, advocacy, and system navigation
  • Knowledge of Developmental Services Ontario (DSO), developmental services, housing resources, mental health support, employment services, education systems, and community resources
  • Strong interpersonal, communication, facilitation, advocacy, presentation, organizational, and problem-solving skills
  • Experience developing community partnerships and collaborating with multiple service providers and stakeholders
  • Ability to work independently while managing multiple priorities and deadlines
  • Proficiency in Microsoft Office and electronic documentation systems
  • Valid Ontario Driver’s License, reliable vehicle, and appropriate insurance coverage
  • Current First Aid and CPR certification or willingness to obtain
  • Flexibility to work evenings and occasional weekends as required

Reporting to the Manager of Development and Engagement, the Community Services Navigator will support transition-aged youth with developmental disabilities and their families as they prepare for and navigate the transition from children’s services to adult developmental services. The successful candidate will provide information, advocacy, education, coordination, and support while promoting person-centered planning, informed choice, community inclusion, and independence.

The Community Services Navigator will play a lead role in the development, implementation, coordination, and growth of a new Transition Youth Program. Responsibilities include transition planning, system navigation, community partnership development, DSO application support, program development, public education, and community capacity building. The position will work closely with youth, families, schools, healthcare providers, community agencies, employers, and government organizations to enhance service coordination and support positive outcomes.

As the program grows, the successful candidate may provide guidance, coordination, and supervision to staff assigned to support the program and contribute to future program expansion.
